This program consists of a wallet size ID containing the child's basic information and a digital photograph. 

 If you would like to obtain an ID, call (814) 776-5353 to make an appointment with  Chief Deputy Merritt.

You will need  the following:

1. Child's Name & address
2. Original birth certificate
3. Social Security Card
4. Signed parent/guardian form (PDF)
5. $5.00 fee (Please make checks payable to "Elk County Sheriff's Office")
